Make sure you're on the latest CWM (Clockworkmod), Unlocked, and Rooted
MAKE A NANDROID in Clockworkmod (As ALWAYS)
wipe data/factory reset in recovery
flash ROM
flash Gapps
reboot

Easiest way to get clockworkmod is to root, download Rom Manager and install it from there. It's never failed me.
