Much of it was based on the fact that Japan's frontline fighters didn't have self-sealing fuel tanks or pilot armor until much much later.

The tanks, and definitely the armour were neglected due to speed and to keep flight characteristics ahead of contenders. it was mistake, especially when it came to that external fuselage tank. the AA rounds are amazing.. even for today. There can be found blueprints online...somewhere?

Out of almost four thousand kamikaze less than one in five managed to hit a ship. If you attack with large numbers it is possible to overwhelm any defense, even proximity armed AA. Without the proximity fuse far more kamikaze would have got through since it improved the accuracy of AA fire by something like 700%.
