Screws are poised to drill into their skulls; they are informed via taped message that the person who cuts off more flesh weight in sixty seconds will live while the other will be killed by the screws. The overweight Eddie cuts fat from his body, but Simone chops her left arm off and tips the scale in her favor, saving herself. Dan Erickson (Mark Rolston) heads up the FBI's investigation into Eddie's death, with Peter Strahm's fingerprints found at the scene. He shares this news with Lieutenant Mark Hoffman (Costas Mandylor), who discovers that Lindsey Perez (Athena Karkanis), previously believed dead from her shrapnel injuries, is still alive and working on the case with Erickson; she was said to be dead in order to protect her from Jigsaw's accomplice. Later, Hoffman meets with Jill Tuck (Betsy Russell) in a private room at her clinic, and she hands him five envelopes from the box from Jigsaw's will in order for him to set up Jigsaw's final game. Hoffman also interrogates Simone in her hospital room, though she is too hysterical to say much. The game revolves around a health insurance executive named William Easton (Peter Outerbridge). His company chooses whom to cover based simply on health and illness, and he turned down two claims that later resulted in the recipients' deaths: Harold (George Newbern) and John Kramer (Tobin Bell). He awakens suspended in a large vise clamped around his torso, a breathing mask covering his face. Hank (Gerry Mendicino), his janitor and a smoker, is in the same situation across from him. William learns that he has sixty minutes to pass four tests in order to retrieve keys to remove bombs strapped to his limbs before they detonate. In the first test, the man who takes a breath causes his vise to tighten; William holds his breath longer than Hank, and Hank's ribcage is crushed. The second tests involves Addy and Allen (Janelle Hutchison and Shawn Ahmed), his elderly secretary and young file clerk; they are both standing on retractable platforms with barbed-wire nooses cinched around their necks. The two are opposites: Addy is ill but has a large family, while Allen is perfectly healthy with no family at all. William must take the ends of the chains connected to the platforms and let one go, allowing that person to die; he chooses Allen, sparing Addy's life. In the third test, Debbie (Caroline Cave), the company attorney, has ninety seconds to make her way through a caged-in maze in the zoo's boiler room to remove a device on her chest set to fire a spear through her head. William must help her from above by diverting jets of steam blocking her path onto himself. At the end of the maze, Debbie learns through X-ray films that the key is in William's side; she tries to cut his side open using a circular saw, but her time runs out and she is killed. The fourth test, a carousel room, has Aaron, Dave, Emily, Gena, Josh and Shelby (James Gilbert, Darius McCrary, Larissa Gomes, Melanie Scrofano, Shawn Mathieson and Karen Cliche), William's junior workers chained to a spinning carousel with a shotgun set to shoot and kill each one at random. William can choose to save two of the six by pushing buttons that will drive spikes through his hand and divert the shotgun upward. He chooses Emily and Shelby, and the other four are killed. After removing the last bomb from his body, he makes it to the end of his path with one second to spare, finding himself between two cages. One contains his sister, Pamela Jenkins (Samantha Lemole); the other contains the late Harold's family, Tara and Brent (Shauna MacDonald and Devon Bostick), who despise William for directly bringing about Harold's death. The test is meant for them, and they can choose to either kill him or let him live. A large switch whose positions are labeled "Live" and "Die" sits in their cage, with a tank of hydrofluoric acid in each cage.
