I recommend using MPC-BE with MadVR video renderer if your computer can handle it. It is the best combo I have found. I like it more than MPC-HC.
 Encoded at 3000 Kbps using x265 2 pass encoding for highest possible encoding. I have compared each and every one of my encodes standing literally at my TV frame for frame (not all of them and pissing my girlfriend off in the process lol) to see if there is any loss and there is none that I can see, so if the quality is not up to par it is the source.
